Before your child can be admitted to class, all forms must be completed and fees paid. Your child will not be allowed to stay at Children's Learning Program until all forms are completed.

All children in Children's Learning Program must be immunized against the diseases of pertussis, tetanus, and rubella, diphtheria, poliomyelitis, measles, and mumps at their appropriate ages. We ask that a copy of each child's immunizations be submitted to the program to keep on file.
